Iran’s newly appointed Supreme Leader, Mojtaba Khamenei, is reportedly unconscious and undergoing treatment in the holy city of Qom, according to a diplomatic intelligence document attributed to the United States and Israel.
The report suggests that Khamenei is currently unable to perform his duties as head of state.
According to the document, his father, Ali Khamenei, was killed in an airstrike on February 28 during the first day of what is described as a U.S.-Israeli military offensive against Iran. Mojtaba Khamenei was later seriously injured in a subsequent strike and is said to be in critical condition.
The intelligence assessment, reportedly obtained by The Times, claims that U.S. and Israeli agencies have, for the first time, confirmed Mojtaba Khamenei’s location. He is believed to be receiving treatment in Qom, approximately 140 kilometers south of Tehran.
The document further alleges that he remains in a critical state and is completely unable to participate in governance or decision-making processes.
It also claims that preparations are underway in Qom for the burial of Ali Khamenei. Intelligence sources have reportedly identified early construction of a large mausoleum, with multiple burial sites possibly intended for members of the Khamenei family, including Mojtaba.
Earlier reports had suggested that the 86-year-old former leader would be buried in Mashhad, his birthplace, but those plans appear to have changed due to security concerns.
Since being named successor in early March, Mojtaba Khamenei has not appeared in public nor delivered any speeches. Only two written statements attributed to him have been broadcast on state television.
The report also claims that an artificial intelligence-generated video has circulated, allegedly showing him in a war room analyzing maps of Israel’s Dimona nuclear facility. However, the absence of his voice in the video has intensified speculation about his medical condition.
Opposition groups claim he remains completely unconscious in hospital. Some reports suggest he suffered severe injuries in the attacks, including broken limbs and significant facial damage.
Analysts warn that the Supreme Leader’s incapacitation has plunged Iran’s political system into deep uncertainty. As the country’s highest religious and political authority, his inability to govern raises concerns about leadership stability. Experts believe the powerful Islamic Revolutionary Guard Corps (IRGC) may now be effectively running the country, with Mojtaba serving only a nominal role.
Meanwhile, U.S. President Donald Trump has stated that discussions are ongoing with other Iranian officials, but no talks or agreements are taking place with the Supreme Leader.
Wednesday will mark the 40th day since Ali Khamenei’s reported death, a significant milestone in Shia mourning traditions. Although burial typically takes place shortly after death, Iranian authorities have delayed the process, citing concerns over unprecedented public gatherings.
Analysts suggest the delay may also reflect fears of potential attacks during funeral proceedings. Authorities are reportedly planning to conduct the burial under strict secrecy to avoid a repeat of the chaos seen during the 1989 funeral of Ruhollah Khomeini.
